Subaxial cervical spine ligamentous instability anterior versus posterior fixation: prospective nonrandomized study

چکیده

Abstract Background The goals of cervical internal fixation are to provide immediate stability control an unstable segment and improve bony union. Early methods preferred a posterior approach consisted simple wire cancellous bone graft. Later, anterior, became recent additions surgeon’s armamentarium. Objectives Comparing between anterior in subaxial ligamentous subluxation regarding the applicability, safety, clinical efficacy achieving enhancing Also cost-related variables studied. Methods This prospective radiographic analysis was performed on 40 patients with subluxation. Half these were treated interlocking screws inter-body cage fusion. other half by lateral mass inter-facet Results There 27 male 13 female mean age 37.4 years. level dislocation C4–5 four, C5–6 14, C6–7 20 more than one two patients. Closed reduction achieved 33 while open through done four cases three cases. Statistically significant difference groups found for estimated blood loss operating room time better results group. no perioperative deaths both groups. Regarding surgery-related complications, there injury root during statistical length hospital stay. stay 7 days. Positioning plate all satisfactory. All followed up at least 6 months. Mean follow-up period Vertebral body alignment (radiological stability) case showed delayed Solid fusion 85% solid 70% fixation. Conclusions Although some literature have indicated that is fixation, yet most studies cadaver subjects so eliminating any fusion, long-term stability, hardware failure. study proved not only safer procedure but also it restores

Posterior Fixation Techniques in the Subaxial Cervical Spine

This article reviews the historical context, indications, techniques, and complications of four posterior fixation techniques to stabilize the subaxial cervical spine. Specifically, posterior wiring, laminar screw fixation, lateral mass fixation, and pedicle screw fixation are among the common methods of operative fixation of the subaxial cervical spine. While wiring and laminar screw fixation ...

Anatomic Study of Anterior Transdiscal Axial Screw Fixation for Subaxial Cervical Spine Injuries

Anterior transdiscal axial screw (ATAS) fixation is an alternative or supplement to the plate and screw constructs for the upper cervical spine injury. However, no existing literatures clarified the anatomic feasibility of this technique for subaxial cervical spine. Therefore, the objective of this study was to evaluate the anatomical feasibility and to establish guidelines for the use of the A...

Anterior Versus Posterior Fixation in Thoracic Tubercular Spine

Tubercular spine has been an age old problem for management. Spine fixation in skeletal tuberculosis has evolved in the last decade, which not only helps in early mobilization of the patient but also prevents late problems of chronic pain and progressive kyphosis. Transpedicular fixation and hartshill loop is done mainly through posterior approach while transthoracic, transmanubrial and transax...

Lateral Mass Fixation in Subaxial Cervical Spine: Anatomic Review

Introduction The cervical spine is a highly mobile segment of the spinal column, liable to a variety of diseases and susceptible to trauma. It is a complex region where many vital structures lie in close proximity. Lateral mass screw fixation has become the method of choice in stabilizing subaxial cervical spine among other posterior cervical fixation techniques whenever the posterior elements ...

Therapeutic strategy for traumatic instability of subaxial cervical spine.

BACKGROUND A simple, safe and effective therapeutic strategy for traumatic instability of the subaxial cervical spine, as well as its prognostic assessment, is still controversial. METHODS The therapeutic options for 83 patients of traumatic instability of the subaxial cervical spine, whose average age was 35 years, were determined, according to the Allen-Ferguson classification, general heal...
